We sent System Design Specialist Travis Hickman to inspect the concrete and see what was causing the cracks. Travis found that the concrete had settled roughly 2 inches and the cracks were caused by this. The soil underneath the concrete had eroded, leaving a void for the concrete to settle into below the surface.
